CHI St. Alexius Health manages four CAHs in North Dakota – Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S. D. Responsibilities Provide Occupational Therapy services using assessment, planning, implementation, and evaluation in a manner consistent with the mission, vision and values of St. Alexius Medical Center. *ESSENTIAL DUTIES AND RESPONSIBILITIES:** *Patient Evaluation*Demonstrates good skill. 2. Demonstrates high levels of efficiency in completion of evaluation and treatments. 3. Effectively considers the patient's age, pain, understanding, communication, social, emotional and educational needs. 4. Performs evaluations and treatment. 5. Development and facilitation of daily OT programming.* *Documentation*Charges accurately reflect the time spent and services provided to the patient. 2. Uses the recommended documentation and charging systems. 3. Effectively documents for quality of care. Review of 5 patient charts demonstrate the following:* Initial assessment documentation* OT assessments measurable (1 functional level)* Goals/kardex noted* Discharge documentation in notes* Documentation OT group programming * *Initiative and Time Management*Demonstrates high initiative and manages time effectively as evidenced by the following: Regularly volunteers to come in extra on weekends and holidays during high work load periods or to cover open shifts. 2. Regularly volunteers to stay late to see late or unexpected patients. Has a plan in place to allow for this and verbalizes it. 3. Actively seeks out other co-workers and supervisor at end of day to provide assistance if they are not finished with patient care. 4. Offers to assist other co-workers and supervisor during times of heavy work loads; i.e. answer phones, retrieve items, etc. 5. Serviced as primary CI for a student affiliation. 6. During slow periods, either leaves work when patients are completed or completes projects and activities, does not sit and wait for shift to end. 7. Informs supervisor when out of office. 8. Provides supervision to COTA. 9. Resource to group techs. * Successfully completed the defined job specific competencies, including competency for age specific, as appropriate. (List the job specific competencies validated during the year in the comment section; at least one must be age specific.) Qualifications *Education: *Graduate from an approved school with a bachelor's degree in Occupational Therapy*. **Licensure*: Eligible for national registration and licensed by the state of North Dakota*Experience: *None*.**Skills: *Entry level skills determined by academic setting the fieldwork experience*Interpersonal Skills: *Relates will to staff, patients and visitors. Maintains confidentiality. Able to communicate and teach patients and family*.**Physical Effort*: Ability to transfer and assist patients with exercise and treatment several times per shift.
